Student Health Information and Enrolment

At Maida Vale Primary School, the health, safety and wellbeing of our students are a priority. We work in partnership with parents and carers to ensure each student’s health care needs are understood and appropriately supported during the school day, including during excursions, camps, sporting events and other school activities.

Our student health care practices follow the Western Australian Department of Education’s Student Health in Public Schools policy and procedures, effective 8 September 2026.

Providing your child’s health information

Parents and carers are responsible for providing accurate, current information about their child’s health care needs and advising the school when circumstances change.

As part of enrolment and when health care needs arise, families may be asked to provide:

  • A completed Student Health Care Summary.
  • Their child’s Australian Immunisation Register (AIR) immunisation history statement.
  • Details of diagnosed medical conditions, allergies and health concerns.
  • Medication authorisation and relevant medical documentation.
  • A management and emergency response plan where required.
  • Information about any health care assistance, specialised equipment or support needed during school activities.

Please inform the school if your child:

  • Has a medical condition, allergy or health concern that may affect them at school.
  • Requires medication during school hours.
  • Has a condition that may require an emergency response.
  • Requires assistance with health care or daily personal care activities.
  • Has a change in their medical condition, medication, dosage or treatment plan.
  • Some health care plans and forms must be completed or signed by a medical practitioner. Please contact the school office if you are unsure which documentation is required.

Keeping health information up to date

Please notify the school promptly if your child’s medical condition, medication, emergency response instructions or support requirements change.

Health care plans should be reviewed at least annually and whenever your child’s needs change. Updated documentation helps ensure staff have the information required to support your child safely.

Parents and carers should also ensure the school has current emergency contact details and can reach an appropriate contact when necessary.

Student health information and confidentiality

Student health information is treated confidentially and managed in accordance with Department of Education requirements.

Information is recorded and maintained securely. Relevant information may be shared with staff who need it to support a student’s health, safety and participation in school activities, subject to applicable privacy and information-sharing requirements.

If you have questions about your child’s health information or the support arrangements in place, please contact the school office.

Please contact the Maida Vale Primary School office to discuss your child’s health care needs, the forms required or any additional support that may be needed before your child attends school activities.
