Thank you for your interest in enrolling your child at Maida Vale Primary School. Parents and carers seeking to enrol a child can complete the Application for Enrolment form below. Please complete one form for each child.
The Application for Enrolment enables parents and carers to express an interest in placement at our school. Submitting an application does not guarantee enrolment; it formally registers your child’s interest.
Applications for Kindergarten Students
Children born between 1 July 2022 and 30 June 2023 are eligible for Kindy in 2027. Applications open on Monday 20 April 2026 and close on Friday 24 July 2026.
Kindergarten Application Requirements:
- Birth certificate (contact the Registry of Births, Deaths and Marriages on 1300 305 021 if you do not have one)
- Australian Immunisation Register (AIR) Immunisation History Statement not more than 2 months old
- Proof of residency (e.g., lease agreement with at least 3 months remaining, proof of property ownership, recent utility bills). Three forms of identity may be requested to verify address.
- Copies of Family Court or other Court Orders (if applicable)
- Information relating to health, medical conditions, disability, or additional needs (if applicable)
- For children not born in Australia or if both parents were born overseas: Visa Grant Notice, passport, date of entry into Australia, and Australian Citizenship Certificate (if applicable)
Please submit the completed Kindergarten Application Form with supporting documents via email to maidavale.ps.admin@education.wa.edu.au or hand it into the school front office.
You will be notified in writing of the outcome within three weeks of the closing date. Once your child is offered a placement, parents will be asked to complete an Enrolment Form to finalise enrolment. Applications received after 25 July 2025 will be considered on a case-by-case basis according to school policy and classroom availability.
In Term 4, parents will be invited to a Kindy information session , providing information on what to expect when your child starts school, including literacy, numeracy, and resilience support. Later in Term 4, a kindy playdate that allows parents and children to spend time in the Kindergarten environment and meet teaching staff.
Applications for Pre-Primary to Year 6 Students
Parents seeking to enrol children in Pre-Primary to Year 6 should complete the Application for Pre-Primary to Year 6 Enrolment Form with the same supporting documents listed above.
Once received, applications are reviewed by the school, and you will be notified of the outcome. Successful applicants will be asked to complete an Enrolment Form to finalise enrolment.
Managing Enrolments
If applications exceed available placements, enrolments are prioritised as follows:
- Children living in the local-intake area with a sibling enrolled, closest to the school
- Children living in the local-intake area without a sibling, closest to the school
- Children outside the local-intake area with a sibling, closest to the school
- Children outside the local-intake area without a sibling, closest to the school
Parents applying for the following year will receive written notification within three weeks of the closing date (end of the first week of Term 3). Applications for the current year will be notified as soon as possible.
